About us

A community where support feels within reach.

The Wellbeing Outreach Team is a non-profit vocational group providing health and social care support to the local community in South London.

Start a conversation

The team was set up by a group of worshippers from the Old Kent Road Mosque who had a particular passion for using their experiences and expertise to serve their local community.

WOT is made up of executive and associate members including nurses, physiotherapists, surgeons, doctors, midwives, pharmacists, social workers, occupational therapists and many more.

Good health, access to public services and support through difficult times are basic requirements that we would all struggle without.

What we do

Support for the real world.

Following consultation with our available healthcare professionals, a referral could be made to an appropriate local healthcare facility such as a GP practice or, depending on severity, an urgent care centre.

Health checks

Multidisciplinary health days and one-to-one consultations for high-risk individuals.

Housing support

Social workers and care coordinators offer advice, support and referral to social services.

Medication reviews

In-house pharmacists and doctors are available to go through medication queries.

Post-discharge check-up

Calls and home visits for high-risk individuals after hospital discharge.

Welfare support

Advice and support with applications to local councils and welfare services.

Whether it is just a chat or a reminder to shut the windows, we are here to be a friendly face.
